Our registered nurses and licensed practical nurses deliver clinical care in your home, working directly with your physician to implement and adjust your care plan.
Important: Home Health Aide services are available only to patients currently receiving skilled nursing services under an RN-developed, physician-ordered plan of care. This is not a stand-alone personal care service.
When a patient is receiving skilled nursing, our Home Health Aides provide personal care support under the direct supervision of an Abia RN.
We communicate directly with your physician, hospital discharge team, specialists, and family members to ensure seamless transitions and consistent care.
